I was riding in this suv with the owner heading down [XXX] towards Highway [XXX] in Madera, CA . We were approaching a turn onto highway [XXX] and there was slowed or stopped traffic in front of us, we slowed to a stop and then was hit very hard from behind by a newer Mustang.ihad my seat belt on, as usual, but we were hit so hard i hit my face on the dash, causing pain and a bloody nose, swollen lips and head pain. The air bag did not deploy, nor did the seat belt hold me in place. We called 911 and had the CHP come out. The car had to be towed away. NHTSA complaint 11768262This incident is in regards to a fire that occurred in the engine bay area, post having turbo replacement and spark plug replacement done by a local dealership where this car was purchased new. Car had roughly approximately 60,000 miles and had a check engine light and engine reduce power notification on dash. Was taken to the aforementioned dealership for general scope of work while in the possession and actively being test driven for the repair the vehicle was driven 5 miles and the driver was flagged down by motorists for smoke. The incident was recorded by Riverside Police Department and Riverside fire department. The origin and caused are still undetermined at this time. Some background in history of this vehicle has never been in a crash and has not had any parts replaced from the original purchase.
NHTSA complaint 11766531Shift to Park error message when car is already in park and then difficulty getting car to turn off. This is an ongoing issue. I have seen online this is a common problem. This is a safety issue because if the car doesn't turn off the battery will go dead leaving a driver stranded. This issue began soon after the car was out of warranty.
NHTSA complaint 11764771While driving to work last Thursday morning suddenly my 2023 Trailblazer let out a sorta growl and then a dragging noise thankfully I was able to get over and stop on the shoulder only to discover that my tail pipe was dragging from just after the catalytic converter as the bolts apparently sheared off leaving only the head of the bolts intact. Lucky for me I there wasn’t major damage. I did however have to get my vehicle towed and have the bolt heads cut off in order to have the exhaust fixed

The contact stated that while his wife was driving at an undisclosed speed, the brake pedal felt extremely firm and was difficult to depress. The contact stated that his wife needed to anticipate the stopping distance to apply enough force to the brake pedal while stopping the vehicle. The vehicle was taken to the dealer on several occasions, but the technicians continuously stated that there was no failure with the vehicle. The vehicle was taken to an independent mechanic and was diagnosed with an inoperable brake master cylinder. The contact was informed that the brake master cylinder needed to be replaced. The vehicle was taken back to the dealer with the information; however, the dealer stated that the brake master cylinder replacement was not necessary. The vehicle remained unrepaired. The manufacturer was notified of the failure. The failure mileage was 40,000.
NHTSA complaint 11759244The shift assembly is going bad and causing a Shift to Park error that GM is aware of. They have issued a Technical Service Bulletin about this exact part. The error code will cause the battery to run down leaving me stranded. The dealer has inspected the vehichle and ordered the necessary part to replace it. The vehicle has not been inspected by the manufacturer, police, insurance representatives or others but is available to have an inspection done. There were no warning lamps for this other than the Shift to Park error showing on the display when the car has been turned off. These error messages started happening around August 1, 2026.

The contact stated that on several occasions while driving at various speeds, the vehicle went into LIMP Mode with the check engine warning light illuminated. The contact stated that on three occasions, the vehicle was stopped and restarted, but the failure persisted. The vehicle was taken to an independent mechanic, where it was determined that the wastegate actuator needed to be replaced, and DTC: P0299 was retrieved. The vehicle was repaired, but the failure persisted. The vehicle was taken to a dealer, where DTC: P0299 was retrieved, and the contact was informed that the turbo or the wastegate actuator needed to be replaced. The vehicle was not repaired. The vehicle was then taken to another independent mechanic, where it was determined that the turbo and waste gate actuator needed to be replaced. The vehicle was not repaired. The contact associated the failure with Manufacturer Recall Number: A222379711. The manufacturer was not notified of the failure. The failure mileage was approximately 33,000.

The contact stated that, while driving on several occasions with the auto start-stop feature activated, the vehicle appeared to shut down at a complete stop as the infotainment screen and dashboard lights turned off. The vehicle restarted automatically but shuddered during startup. The dashboard lights and the infotainment screen became operable. The vehicle was not diagnosed or repaired. Additionally, the contact stated that while driving at approximately 30 MPH, the thermostat needle went to Hot, and the Check Engine warning light illuminated. The vehicle was steered to the side of the road and turned off for approximately 15 minutes. Upon restarting the vehicle, the check engine warning light remained illuminated. The vehicle was taken to an independent mechanic, where it was diagnosed that the coolant flow control valve had failed and needed to be replaced. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 88,251.

The contact mention she pulled off the interstate, turned the vehicle off for five to ten minutes then turned the vehicle back on and the vehicle began to work as intended. A dealer was contacted. The vehicle was taken to a dealer where it was diagnsonesd the turbo booster needed to be replaced. The vehicle was not repaired. The manufacturer was notified on the failure, and they informed her there were no recalls at this time. The approximate failure mileage was 103,000.

The contact stated that while parking the vehicle, the computer system failed to recognize that the vehicle had been shifted into the park position, causing a "Shift to Park" warning light to illuminate on the instrument cluster. Although the gear selector was physically in park, the contact was unable to fully shut down the vehicle and had to cycle the ignition on and off multiple times until the system eventually recognized the park position. The vehicle was driven to the dealer where it was diagnosed to be a transmission control assembly failure. The vehicle was repaired. The manufacturer was not notified of the failure. The failure mileage was 61,097.
NHTSA complaint 11736042While driving on the highway in light traffic at approximately 70 mph and changing lanes, the side airbags in my vehicle suddenly deployed without warning. There was no collision, no contact with another vehicle, no road debris, and no apparent external cause for deployment. I did not strike anything before, during, or after the deployment. There were no vehicles within several car lengths of my vehicle at the time. Highway Patrol was dispatched to the scene. Officer TJ Carter with Guilford County responded. Upon inspection of the vehicle at the scene, we found no evidence of impact or contact with another vehicle, object, or debris. The vehicle was covered in yellow pollen at the time, and even light contact would have left visible marks or disturbance in the pollen. After examining the vehicle, no signs of contact were identified. The vehicle does have unrelated prior body damage from over a year ago on one side, but that damage predates this incident and was not caused by this event. The responding officer can confirm there was no collision or new impact associated with the airbag deployment. There was no accident associated with this airbag deployment. The airbags deployed while I was traveling at highway speed, which caused a sudden and dangerous driving event and required me to quickly maneuver to the side of the roadway. This created a serious safety risk to me and to surrounding traffic. Prior to this incident, there were no warning lights, safety alerts, or service notifications indicating an issue with the airbag system or vehicle safety systems. However, a notification did appear in the My Chevrolet app at or around the time of the incident. The vehicle has not yet been fully inspected following the incident. A safety claim has been submitted to General Motors. I also have video footage of the Highway Patrol Officer cutting down the airbags on-site but cannot add that below so I have added a screenshot.
NHTSA complaint 11727596The bolts holding the exhaust to the catalytic converter broke while I was driving my car. The car could have flipped, the exhaust could have busted through the vehicle, or I could have caused an accident and died. This comes on the heels of the EXACT SAME THING happening to my wife's Chevy Trailblazer with nearly the same mileage as mine. I have 97K miles and she 96K. This is a KNOWN ISSUE to them and it is NOT THE FIRST time this has occurred. There are thousands of similar instances that have occurred and one can easily find the anecdotes through an online search.
NHTSA complaint 11724711The Onstar module has went out on my car and while researching the issue I found out that it is a very common problem with Chevrolet/GM vehicles years 2021-2025 and although Chevrolet and GM are wwll aware that there's an issue and that the onstar modules are faulty there's been no recall. Mind you the module doesn't just affect onstar features it also is connected to bluetooth microphone and remote start and now none of them work at all.
NHTSA complaint 11724012Screen in car goes black.no radio,no noise,no back up camera-Apple car play won’t hook up anymore-I see others models around mine having exactly the same issues I am experiencing-I have reset radio/screen multiple times,it works anywhere from a few mins to hours then it blacks out again and there is nothing on screen.i have replaced the battery,key fine battery’s,and fuse.and I am still experiencing this issues.i have made an appointment at a Chevy dealer to have it diagnosed March 2,2026.that is $169.00 out of my pocket.again I am seeing plenty of other checks around my make and model having the same issues being recalled. My incident started about January 30,2026.

Hewas in the process to start conditioning for football. The seatbelt on the day of the accident did not lock. GM sent a criminal investigator not a trained engineer for the vehicle. The seatbelts needs to be inspected. The motor/transmission is making a ticking sound.
