The contact stated that the battery had previously been replaced. The check engine warning light had illuminated. The local dealer was contacted, but the vehicle was not diagnosed or repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the failure. NHTSA complaint 11590061The car's lithium batteries caught fire to the point of smoldering. This happened at the end of the charge cycle. The car was parked in our garage and if the fire department did not come to help, the fire could have intensified and burned down our whole house. We are very thankful we caught it so quickly. No warnings or issues were apparent with the vehicle when it was parked and put on the charger.
NHTSA complaint 11558139What component or system failed or malfunctioned, and is it available for inspection upon request? - Internal charging system or vehicle battery pack malfunctioned & caught fire. Vehicle is available for inspection now, but not sure about when it is removed from our property. How was your safety or the safety of others put at risk? - It was a big fire! Close to structures, vehicles & dry foliage – super lucky it didn't spread. Smoke was very noxious and affected our entire neighborhood. Neighbors have reported respiratory irritation since the fire. I now have a cough, burning throat and headaches. Has the problem been reproduced or confirmed by a dealer or independent service center? - There's a burned out van at the end of my driveway... specific instance of this problem does not need confirmation Similar issues are reported on the internet. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? - Not yet. Chrysler says it'll take 2-3 days for issue to be "escalated". Insurance is sending someone. Fire department HazMat crew verified that the battery pack is now "cold" but toxic chemicals are a public safety hazard. Oakland Fire incident #[XXX]. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? - July 2022 the vehicle refused to charge & displayed a "turtle" icon. Took it to the dealer and told of the charging/fire recall. Stopped charging until all the recall parts were available which wasn't until August 2023. After repair, we were told the car was OK & it was safe to resume charging the car. Other details: - The vehicle caught fire while charging, engulfed in flames, had to be extinguished by Fire department, total loss. Car plugged in at ~4:30pm and we heard a "pop" at ~8:20pm. So the battery had reached 100%. Ran out and there was small fire dripping from middle underside of the car– quickly spread & consumed the entire car INFORMATION REDACTED PURSUANT TO THE FREEDOM OF I
NHTSA complaint 11553283The driver-side rear sliding door is not closed securely and it reopened while closing without any other button access and obstacles on the door. But seems like it was fixed with an ECU software update. I have a problem with that and it was fixed with a repair visit last week. That means it was a known issue, but I must pay the repair cost. I have a short video clip to show the problem and my dealership was able to reproduce same symptom while they doing a diagnosis.
NHTSA complaint 11525468The vehicle's high voltage battery was being charged using a Siemens VG2 EVSE when an arcing sound was noticed and burning smell and smoking was seen. Looking back at our home energy monitor, it appears to have been arcing for close to an hour without any warnings being thrown until shortly after being noticed. The car did eventually show a charging system error, but the charging didn't stop until I physically unplugged it. The L1 pin on the vehicle and the EVSE showed scorch marks and damage from the arcing, but thankfully it was caught before a fire happened.
NHTSA complaint 11523391I have been trying to take care of this urgent safety recall since February 2023. The local dealership (4520 Shelbyville Rd, Louisville, KY 40207) keeps telling me that they don't have a "loaner" care to provide to me while my vehicle is being repaired. I am a single father with 7 year old and primary care giver to 77 year old mother with Alzheimers, I must have a replacement car while mine is being repaired especially since this is the manufacturer responsibility. Please help.

The contact stated while driving 75 MPH, the check engine warning light was illuminated, and there was a burning odor coming from the A/C vents. The contact veered to the side of the road and noticed that the burning odor was also coming from underneath the hood of the vehicle. The vehicle was taken to an independent mechanic where it was diagnosed that the catalytic converter and oxygen sensors needed to be replaced. The contact stated that the vehicle was previously repaired under NHTSA Campaign Number: 18V740000 (Hybrid Propulsion System, Engine and Engine Cooling) in 2018. The contact stated that the recall remedy was not effective. The vehicle was taken to the dealer where it was diagnosed; however, the dealer declined to provide diagnostic information to the contact after several attempts. The vehicle was not repaired. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was approximately 68,849.
NHTSA complaint 11501537We had the FCA Recall #Z11, NHTSA Recall #22V-077 repaired on this vehicle. One week later, the plug-in hybrid Pacifica was parked in the garage, plugged in but done charging. In the middle of the night, the battery spontaneously combusted. We heard a loud boom sound and found the car smoldering and emitting significant amounts of smoke. The car then burst into flames. The fire department soaked the car with water but it continued to reignite for approximately two hours despite the fire department’s best efforts to extinguish the fire. The car was completely destroyed. It also caused significant chemical smoke damage to our garage and home. As soon as we found it smoking, we rolled it out of the garage, which saved our house from catching on fire. Luckily our children were not in or next to the car at the time the battery combusted. There were no warning signs in the car prior to this incident. The vehicle has been inspected by insurance and is available for further inspection. At least two members of the family experienced adverse health consequences following the smoke exposure.

The contact stated while driving 55 MPH, the battery level was low and the warning message stating to plug in the vehicle to charge and to decelerate was illuminated. The vehicle stalled and lost motive power; however, the contact was able to pull over. The contact attempted to restart the vehicle several times. The contact stated that the vehicle started up for several minutes but then stalled. The vehicle was towed to the dealer and the contact was informed that the vehicle needed to be charged for a full day, but no failure was found. The mechanic at the dealer had experienced the vehicle unintendedly accelerating. The dealer stated that the failure would be reported to the manufacturer. The manufacturer was contacted but stated that there was no record of the failure. The failure mileage was 39,947.
NHTSA complaint 11495433We recently took our Hybrid Chrysler Pacifica to the dealer and they mentioned that there are multiple engine and transmission mounts that need to be replaced in addition to the entire transmission. I reported to them over the years that the vehicle stutters up steep hills (>13.5 degrees). The electric motor produces too much torque, lurches forward, then cuts off and repeats the process. In doing so the electric drivetrain motor is twisting the vehicle apart. It is certainly a safety issue. If we get it fixed, it will certainly happen again and to anyone that has a steep driveway or goes up inclines at less than 5 mph. The process is easily repeated. Thank you!

NHTSA complaint 11482668Recall over 6 months without resolution leaves the vehicle unusable under the conditions it was sold by manufacturer. The inability to use a plug-in hybrid as a plug-in hybrid renders it not functional for the short daily drive with the electric motor.
NHTSA complaint 11482133Recall Z11 was announced in early February 2022 due to fire risk which prompted Chrysler to issue a recommendation that owners not charge their plug-in hybrid Pacifica Hybrids and to park them outside until the fix was completed. More than six months have passed without subsequent update to resolve this recall, and not only is no part or remedy identified, the underlying cause has yet to be identified. This is after two previous recalls, U94 in October 2018 and W46 in June 2020, which were also announced by Chrysler due to fire risk for the Pacifica Hybrid. Chrysler's guidance of parking the vehicle outside and not plugging it in to charge is unsatisfactory and illogical. The normal operation of the plug-in hybrid system leads to the gasoline engine and regenerative braking charging the high-voltage battery during its "Hybrid Mode" operation even when the vehicle has not been plugged in to charge the high-voltage battery. Based upon other recent fire risk recalls related to vehicles with high-voltage batteries such as the Chevrolet Bolt EV--which has led to numerous manufacturer repurchases and complete battery replacement for all affected models--Chrysler's response is neither timely nor sufficient. It is past time that definitive resolution be provided to owners of vans that have already been affected by two prior fire risk recalls and now have been waiting six months with recommendation to not use the vehicle as designed, marketed, and advertised at the time of sell. Given the unresolved nature of recall Z11, I request action be taken to require manufacturer buybacks of the affected products at this time.
