  1. Filed Sep 17, 2026Failure Sep 12, 2026West Melbourne, FLElectrical system, Service brakes, Unknown or other

Incident: On Sept 12, 2026, my 2024 Toyota Prius Prime continued moving forward after I pressed the Park button, causing a collision with my condo garage wall. This was unintended vehicle movement after Park engagement. The vehicle uses a push‑button, shift‑by‑wire Park system, so any electronic malfunction can affect its ability to remain stationary. Recent recall work: Two days before the incident (Sept 10, 2026), Toyota performed recall work on the vehicle. Because the Prius Prime relies heavily on electronic control modules, this timing may be relevant to the malfunction. Same day: The vehicle displayed “No Key Detected” even though the key fob was inside. This may indicate intermittent electronic or communication faults involving the BCM, key‑recognition antennas, or shift‑by‑wire system. Prior to incident: The vehicle abruptly switched from EV to gasoline operation despite having substantial battery charge and under light driving conditions. This behavior is inconsistent with normal Prius Prime operation and may indicate instability in the hybrid control or battery‑management systems. Past electronic malfunction: On March 1, 2024, after service at Toyota of Melbourne, the automatic headlights and driver‑assistance features were not functioning. The service invoice (RO #TOCS730453) documents these issues, showing prior electronic system irregularities. Toyota response: Toyota acknowledged my claim on Sept 15, 2026 and opened Case #260915003140, forwarding it to their field office for review and possible inspection. Request: I am reporting this as a potential safety defect involving unintended movement after Park engagement, recent recall work, and intermittent electronic malfunctions affecting multiple systems.

This is a 2024 Toyota Prius prime lease in February 2024. I went to our trusted local car shop for our annual inspection, and was told that we need to replace all four tires and we’re just passed 24,000 miles on our odometer. We are two seniors living in New York State and we only drive on paved roads. We believe this is a failure both of Toyota and Toyota for choosing these tires on this particular model. We were told we need to spend over $1000 to replace tires on a car we do not own. Toyota pointed us in the direction of Toyo, and Toyo pointed us in the direction of Toyota. This is unacceptable and we do not want to pay $1000 for tires that should not be worn out after 24,000 miles with normal driving. We’d like to get some satisfaction on this issue.

Although the involved vehicles are within the above production period range, not all vehicles in this range were sold in the U.S. This issue only affects 2023-2026MY Prius and Prius Prime vehicles equipped with electric rear door lock system with a door opener switch manufactured by a specific supplier and during a specific production period. Other Toyota and Lexus vehicles sold in the U.S. are either equipped with a different electric rear door lock system or do not have an electric rear door lock system and do not have the condition described in this report. Unknown. Toyota is unable to provide an estimate of the percentage of vehicles to actually contain the defect. Whether water enters the door opener switch and causes a short circuit that could lead to a door switch activation depends on various conditions, as described further below. However, as the NHTSA manufacturer portal requires an integer value be entered, Toyota has entered the value “1” in response to this question in the portal. For the purpose of this report, “1” means “unknown”.

  6. Filed Jun 9, 2025Failure Jun 8, 2025Grover Beach, CAElectrical system

Since Nov 23, 2024 I have had three occasions where the 12 volt battery for the ICE has depleted leaving me stranded. The only indication of the problem is the car will not start. In my last incident I had to charge the battery (disconnected from the car) but inadvertently closed the hatch. Since the car uses electronic locks the only way to open the hatch is to use the emergency release that is located under the rear deck and must use a screw driver to release. This is inherently dangerous since no practical escape possible in an emergency.

  7. Filed Jun 4, 2025Failure Apr 15, 20253,500 milesLivingston, TXWindshield wiper/washer

The contact owns a 2024 Toyota Prius Prime. The contact stated while driving 65 MPH, the windshield washer and wipers failed to function as intended. The contact drove the vehicle to the shoulder of the road and physically removed mud from the front windshield. The contact stated that an oversized truck hauling mud splashed the vehicle and mud landed on the front windshield. The contact stated that the windshield washer volume and speed was insufficient to clear the windshield. The contact stated that because of the low windshield washer sprayer speed and volume of the washer fluid, the contact feared for his safety. The vehicle was taken to the dealer; however, the failure was not duplicated. The contact was informed that the vehicle operated as designed. The manufacturer was made aware of the failure but provided no assistance. The failure mileage was approximately 3,500.

Car with only 7000 miles on it. Took to dealer for low 12 volt battery warning and was told nothing was wrong a few months ago. Was stranded with vehicle in 95+ degree heat when 12volt battery went dead and Toyota Care said it would be 90 minutes before help could come. This is a SAFETY issue. I’m [XXX]. It’s not a good thing to be stranded in the heat. Many 2024 owners are having this problem. They have had to carry a jumper with them at all times. This car is a lemon and Toyota is just gaslighting its customers. Please get on social media and read about all the problems with the 12 volt battery on this car. I can think of many scenarios where being alone stranded with a dead car could be dangerous. Thank you. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
