* The suspension camber is extremely off from the factory * The tires are wearing abnormally, causing the steel mesh to become exposed while the tire still appears healthy from the outside * Yes, the problem has been reproduced, and is widespread * Yes, it's been inspected by the dealership, and Toyota corporate is doing everything they can to deny fixing it. * There were no warnings, and despite almost a dozen visits to dealerships for service, no one brought it up until I was outside the warranty coverage time.
NHTSA complaint 11768219I was stopped for one minute in a drive-thru, and when I took my foot off the brake to accelerate, the vehicle stalled. It told me "Engine Stopped Power Steering Low". I moved out of the drive-thru in neutral, turned off the car then turned it back on. Everything was okay and had no issues. At the time of the accident, my windshield wipers were on. My eninge's auto stop/start was also on. There was a safety risk because my car stalled out when starting up, causing me to have to turn off the car and turn it back on while stopped. I have not been able to reproduce the problem with my dealer yet. I have not had the vehicle inspected by the manufacturer, police, or insurance representatives? I did not have any warnings of this incident. I got a "Engine Stopped Power Steering Low" message when the issue occurred.
NHTSA complaint 11765464forward recognition camera malfunctioned which turns off all front camera associated systems including the entire toyota safety sense system. Precollison system malfunctions, lane tracing, adaptive cruise, and headlight system all malfunction when this camera is out. The car is only 2 years old and had to have the entire camera replaced for $2,000.
NHTSA complaint 11764251On 9/06/2026 my 2024 Toyota RAV4 Hybrid was involved in a severe collision where the impact occured on the drivers side at the passenger door. The vehicle spun several times before coming to a stop. During the spins the front passenger door flew opening. My wife was sitting in the passenger seat her seatbelt successfully restrianted her preventing ejection and injury. However the unlatching of a non impacted door under rotational forces represents a catastrophic structural or mechanical latch failure that poses an imminent risk occupant ejection.
NHTSA complaint 11762632I own a 2024 Toyota RAV4. At approximately 38,300 miles, an authorized Toyota dealership diagnosed a failed rack-and-pinion/steering rack assembly that required replacement. Before the repair, I began noticing a squeaking noise when turning the steering wheel. The noise became progressively louder over time. The symptoms continued to worsen until the vehicle was inspected by the Toyota dealership. The vehicle has been regularly maintained, and there was no collision or other known event that would explain this failure. The steering rack required replacement only about 2,200 miles beyond Toyota’s 36,000-mile basic warranty. Because the rack-and-pinion assembly is a critical steering component, I am concerned that this may represent a premature failure and potentially a safety issue if similar failures are occurring in other 2024 Toyota RAV4 vehicles. I am submitting this complaint so the issue is documented and can be compared with reports from other owners. I have dealership repair documentation confirming the diagnosis and replacement of the steering rack assembly.

NHTSA complaint 11758696The contact owns a 2024 Toyota Rav4. The contact stated that while driving approximately 2–4 MPH, the vehicle independently accelerated. The brake pedal was depressed; however, the vehicle failed to stop as intended. The vehicle was uncontrollable. The vehicle drove onto a sidewalk curb and crashed into a concrete post before coming to a stop. There was no damage to the concrete post. The contact stated that the front camera and traction malfunction error messages were displayed. The contact stated that the failure had occurred while the contact was driving in a parking lot and attempting to park the vehicle. The vehicle was towed to a local collision shop. No Police report was filed. The contact sustained injuries to the upper chest from the impact and the seat belt. The contact had an upcoming appointment with the primary care physician. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 51,020.

The contact stated that the Headlight System Malfunction, Pre-collision System Malfunction, Road Sign Assist (RSA), and Lane Tracing Assist (LTA) warning lights were illuminated. The vehicle was taken to the dealer, who was unable to diagnose the vehicle. The contact stated that the remedy for Special Service Campaign: 26TC03, which addressed an incorrect MAF Sensor reading failure, had been performed two days prior to the failure. The vehicle was not repaired a second time. The manufacturer was informed of the issues. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 48,000.
NHTSA complaint 11756414Airbags, seats, and headrest all failed. State Farm insurance currently has the vehicle. I do not know if the people has been inspected by anybody. The vehicle had no warning lamps message or other symptoms of problems or failure before the collision. I was traveling in the far left lane when traffic came to abrupt stop and I was able to stop 10 to 15 ft from the vehicle in front of me. Two vehicles in my line behind me swerved out of the way. Then a semi truck with a trailer rear-ended me. The front end of the semi truck struck the rear end of my vehicle getting lodged inside my vehicle. My car was then pushed into the car in front of me and dragged 100 to 200 yards down The highway by the semi. The seat belts did not lock. The airbags did not deploy. Both front seat frames were bent and ripped apart. The rear seat that's became detached. The headrest for the front seats flew off. All four seats were occupied by adult males all weighing around 190 pounds or above. All four occupants were wearing seat belts. I the driver was knocked unconscious and received a concussion plus facial damage/injuries. Rear passenger side occupant received a broken jaw and a skull fracture. The other two passengers were also injured. All four passengers of the vehicle were transport to emergency room via ambulance.
NHTSA complaint 117561672024 Toyota RAV4 Hybrid Limited : issues with vehicle detection but not limited to the Blind Spot Monitoring System (BSM) . Complaint includes problems with the vehicle's sensors and electronic control units (ECUs), which is leading to difficulties in detecting other vehicles on the road and posses ongoing safety concerns about the reliability of my vehicle's safety features, particularly when driving my car on freeways and surface streets. Sensor Malfunctions: I continue to have malfunctions with the sensors that are supposed to detect nearby vehicles. I have taken my vehicle into Wilsonville Toyota’s Service Center in [XXX] twice, once in XXX and again in XXX after having at least two near collisions due to the fact that my car did not detect an approaching vehicle. On one occasion my friend ([XXX]) was in my car with me when we almost hit another car when changing lanes on the freeway because my car DID NOT DETECT a vehicle in the lane I was attempting to change. I feel unsafe due to the inconsistent performance of my BSM System Malfunction. I have visited my Toyota dealer for diagnostics and potential software updates but have been told repeatedly that if they, Wilsonville Toyota Service Center (WTSS) cannot reproduce the issue then it does NOT exisit. However, after having and picking up my vehicle from the repair shop at WVTS on XXX I spoke with the acting Assistant Service Manager, Kevin, who stated to me me that if it happens again to return and he will go for a test drive with me to see if I could reproduce the situation so that he could see it and make documentation of it so that it will then be “a thing”. Well I did just that on XXX when my husband , [XXX] , and I returned to WVTS and took Kevin for a test drive where Kevin experienced my exact concerns with the BSM’s Malfunction on more than 4 times on our short test drive. NHTSA complaint 11752528When using the CarPlay feature I am not able to use the maps. When using the maps the it glitches out and constantly reroutes spastically and feels very unsafe while driving. I looked up if this was a common issue with other 2024 rav4 owners and I learned it is. It was explained as an issue with the antenna not being able to keep up with the location of the phone. This issue has caused me to not be able to use my screen while driving and defeats the whole purpose of having that feature in the car. I tried calling a Toyota dealership to fix the problem but was told I would need to pay 200 dollars just to get the car looked at and being as this is a defect with the vehicle that feels unfair. I appreciate any assistance you can offer to resolve this issue for myself and any others experiencing the same problem.

The contact stated that while driving approximately 30 MPH in a neighborhood, an advertisement unexpectedly appeared on the center display and obstructed the navigation screen. The contact stated that on the most recent failure, the navigation screen returned after a short while; however, on a previous occasion, the navigation screen could not be restored without restarting the vehicle. The contact expressed concern that the condition prevented access to the navigation system while driving. The vehicle was taken to a dealer; however, no cause for the failure was found. The vehicle was not repaired. The manufacturer was notified of the failure; however, no response was received. The failure mileage was approximately 17,500.

The contact stated that while driving approximately 55-60 MPH on a freeway, the blind spot monitoring system intermittently failed to detect vehicles in adjacent lanes. The contact stated that the failure nearly resulted in two crashes. There were no warning lights illuminated. The contact continued driving the vehicle to the residence. The vehicle was taken to a dealer within one week for inspection; however, the dealer was unable to duplicate the failure and no repair was performed. The contact stated that the failure recurred, and the vehicle was returned to the dealer, where it remained. The vehicle had not been diagnosed. The manufacturer was notified of the failure; however, no update was provided. The failure mileage was approximately 18,000.
NHTSA complaint 11746514I was in a drive thru and my car had been idling for less than 5 minutes in the rain and started shuttering really bad with my roms dipping low. i put the vehicle in park and the the entire vehicle shut off and would not stay on without dying. for the next 20 minutes everytime i tried to start it it shut off immediately no matter what i would do. the vehicular kept telling me “Engine Stopped steering power low”. it let me put the vehicle in accessory mode and was fine but my steering wheel remained locked. after about 30 minutes i was able to get the vehicle to start and pull up about 15 feet before it shuttered and i put it in park and it died again. i had to get it towed to the dealership.
NHTSA complaint 11745756I purchased a 2024 Toyota RAV4 in March 2024 with the expectation that its advertised safety and driver-assistance systems would function as designed. The vehicle has experienced a failure of the forward camera/ADAS system, resulting in the loss of multiple Toyota Safety Sense features. The following systems are currently unavailable or malfunctioning: • Dynamic Radar Cruise Control / Adaptive Cruise Control • Pre-Collision System • Lane Tracing Assist / Lane Keeping Functions • Road Sign Assist (speed limit recognition) • Related driver-assistance and safety warning functions When cruise control is activated, the vehicle displays a warning that cruise control is unavailable. The dashboard reports multiple safety-system malfunctions. As a result, I have lost essentially all advanced driver-assistance features that were included with the vehicle. Toyota’s dealer diagnosed the failure and Toyota approved the repair under a goodwill case. However, I was informed that the required replacement component is on national backorder with an estimated wait of approximately two months and no guaranteed delivery date. Despite acknowledging the defect and approving repair, Toyota has not provided a comparable loaner vehicle or alternative transportation. I am being asked to continue operating the vehicle for an extended period without the safety technologies that were a significant factor in my purchase decision. I am concerned about the loss of collision-avoidance, lane-assistance, and adaptive-cruise functions, the indefinite repair delay, and the lack of transportation assistance while awaiting parts. I am requesting investigation, assistance, and consideration of repurchase or other appropriate remedies.
