The foam under the headrest was discovered to be cracked when the cover was removed while performing the the repair on the recall for the babies eating the foam (can't find the recall paperwork now). The instructions provided with the remedy for the recall said if the foam was cracked to contact evenflo's hotline immediately. I contacted evenflo on July 28th via phone and was emailed a request for more information. I provided more information and video of the damage on XXX. No reply was received acknowledging the receipt of the requested information so I emailed on XXX asking to confirm receipt. I emailed again on XXX, and Evenflo finally responded on XXX with the following reply: "Thank you for reaching out to Evenflo regarding your Revolve360 car seat. I'm sorry to hear your headrest foam has become broken/detached. If the foam is detached, tape is allowed and no glue, and the seat pad holds the foam in place, the seat is still safe to use. If the foam is broken in larger pieces, you have all the pieces, they fit together like a puzzle, tape allowed and no glue, and the seat pad holds the foam in place, the seat is still safe to use. Evenflo tests all car seats to energy levels approximately twice that of the federal crash test standard. Additionally, our engineers developed the Evenflo Side Impact test protocol, which simulates the energy in severe 5-star government side impact tests conducted for automobiles. Now we are leading the industry by incorporating a dynamic rollover test. Rest assured, as long as the foam is in place, broken or not, it is still doing its job of absorbing crash forces." I no longer feel safe using the carseat with my [XXX] and wanted to reach out to the NHTSA to verify that cracked headrest foam is safe to use. NHTSA complaint 11766516The contact owns a 2025 Toyota Sienna Hybrid equipped with Firestone Tires, Tire Line: FT 140, Tire Size: 235/65/R17, DOT Number: 1WT87FT01. The contact stated that there was a screw in the tire. The contact stated that while driving 70 MPH there was a sound of an object in the tire. The message that the air pressure was low was immediately displayed. The tire pressure indicated 25 PSI. The driver was able to take the off-ramp but was unable to locate the spare tire. A good Samaritan attempted to assist. The driver was able to locate the Fix-A-Flat and a compressor, but the tire failed to hold air. The contact stated that the hole in passenger’s side rear tire was too big to be sealed. The contact was unaware that the vehicle did not have a spare tire. The contact was on the side of the road for two hours. The tire was changed and plugged by her husband after two hours of being stranded. The contact had enough fuel to run the air conditioner. The tires were previously rotated. The contact stated that the situation could have been worse, and not having a spare tire puts the drivers in a dangerous situation. The failure mileage was approximately 11,474.

NHTSA complaint 11760919While driving my 2025 Toyota Sienna XLE with Sure Grip Push‑Rock electronic hand controls, the acceleration suddenly stops working without warning. When this happens, the hand control will not accelerate at all — only the brake still works — leaving me coasting in traffic with no way to speed up. This has happened multiple times at different speeds and creates an immediate danger because I can only slow down or stop, not accelerate out of the situation. At the same time these acceleration failures occur, the vehicle shows a “SRS Airbag System Malfunction” warning. Toyota’s internal records show the airbag system had faults before I bought the vehicle. The airbag warning and the acceleration failure often appear together, which makes me believe there is an electrical or wiring problem affecting multiple systems. I also discovered that Toyota documented underbody impact damage from a pothole months before I purchased the vehicle. This damage was not disclosed to me, and CARFAX does not show any record of this damage, even though Toyota’s own service history confirms it. The CARFAX report I received states “No accidents or damage reported to CARFAX”. I have owned multiple BraunAbility‑converted vehicles, and all of them have had electrical issues. BraunAbility cuts out the bottom of the vehicle for the lowered‑floor conversion and splices in their own wiring harness, which I believe is causing repeated electrical problems across different vehicles, including this one. Because the acceleration fails, the airbag system shows active faults, and the wiring appears to interfere with the vehicle’s safety systems, I am reporting this as a serious safety hazard.

NHTSA complaint 11752616We purchased a used 2025 Toyota Sienna a few weeks ago. The other day, my husband put his phone and wallet into the center console compartment and closed the lid. When we went to retrieve those items, the lid only opened partially. We were finally able to remove the wallet but the phone had slid down into the compartment where the door to that console opening slides into when you open and close it. The lid would not open all of the way and blocked our access to that space when you opened it. The only way to remove the phone was to take it to our local dealership where they spent hours removing seats and pieces of the console to remove the phone from the inaccessible area and then put everything back together. This cost us roughly four hours of work time and $311.99 out of our own pocket. Apparently this is a known issue with this vehicle and is a design flaw where items of the right size (including iPhones) can slide into that compartment while the lid is closed and then are inaccessible.

NHTSA complaint 11747644The Supplemental Restraint System (SRS), specifically the airbag system, is the component of concern. During a two-impact collision involving my 2025 Toyota Sienna XLE, none of the airbags deployed despite significant front-end damage that resulted in the vehicle being declared a total loss. The vehicle is currently in the possession of the insurance company or its storage facility. I do not know whether it has been released for salvage, but it may still be available for inspection. I, my wife, and our children were all wearing seatbelts at the time of the collision. After an oncoming vehicle struck the front-left portion of my vehicle while making a left turn, my vehicle lost control and collided with a concrete pillar. None of the airbags deployed during either impact. All occupants sustained injuries and were transported to the emergency department. I suffered injuries to my right leg, wrist, chest, neck and back. My wife sustained injuries to her neck, chest, back, and knee, and our children sustained injuries including head impacts. My wife and I continue to receive physical therapy. The vehicle was declared a total loss following the collision, and no dealer or independent repair facility has evaluated or reproduced the concern. The vehicle was inspected by the responding law enforcement officers at the crash scene, and a police crash report was completed. The insurance company (GEICO and Allstate Fire and Casualty) inspected the vehicle and declared it a total loss. To my knowledge, Toyota has not yet inspected the vehicle, and I do not know whether the Event Data Recorder (EDR) or Supplemental Restraint System has been analyzed. Prior to the collision, I did not observe any airbag warning lights, SRS warning messages, or other indications that there was a problem with the Supplemental Restraint System. The vehicle was operating normally before the crash.

NHTSA complaint 11743368June 3, 2026, approximately 5:30 PM EDT, I entered my 2025 Toyota Sienna to move it from the street beside my home, to my driveway. Upon closing the driver's door, the rear liftgate window exploded into the vehicle. This occurred without any warning or adverse action by myself or anyone else. The vehicle has been in no collisions, and has only 6429 miles on the odometer. I phoned Toyota's "Customer Experience Center" to report the incident and to seek warranty service for this problem. I was told to drive the vehicle to my Toyota Dealer, which is located 115 miles away, for inspection. I explained that it is unsafe to drive a vehicle filled with broken glass and with no rear window 115 miles. If I vacuum up the glass, there is nothing for the dealership technician to inspect. If I tape plastic over the opening, it could come loose at highway speed, and cause an accident, and the tape adhesive could damage the vehicle finish. The Toyota agent advised me that the vehicle must be inspected at a Toyota Dealership, and if the vehicle is unsafe to drive to the dealer, we could have it towed. But in a subsequent call to the "Customer Experience Center", my wife was informed that Toyota would not cover a tow of that distance in their warranty, and it was our fault for buying a car and taking it to an area that far from a dealer. The phone number we called is located inside the vehicle owner's manual cover under a heading, "If there is not a Toyota dealer near you, please call the following number:" In the morning, once they are open, I will call the nearest dealership, 115 miles distant, to seek further guidance. Based on my background in both mechanical engineering and automotive failure analysis, it is my opinion that the window and its support system was not designed to withstand the prolonged sun and heat exposure experienced. The outside temperature reached at least 82 degrees F where the vehicle was parked.

We have not been able to use the second row seats for over 7 months, and there have been no communication or updates from the manufacturer regarding a remedy. Safety Risk If these seats are occupied during certain high-speed collisions, the seat may lose structural integrity, increasing the risk of injury. Remedy Toyota is currently preparing the remedy. When the remedy is available Toyota dealers will replace the second row seat rails with ones that have the proper welding, FREE OF CHARGE.
