    The contact owns a 2010 Lexus LS460. The contact stated while driving approximately 65 MPH, the brake pedal was depressed and extended to the floorboard. The contact stated that the ABS, brake, and Traction Control warning lights were illuminated. The vehicle was taken to the local mechanic who diagnosed that the ABS module was faulty and needed to be replaced. The vehicle was taken to the local dealer to be diagnosed. The vehicle was not yet repaired. The manufacturer was not contacted. The failure mileage was approximately 170,000.

    The contact owns a 2010 Lexus LS460. The contact stated that while driving at 20-25 MPH, the warning message 'Have Your Dealer Check the VGRS System" was illuminated. The contact stated that he also noticed that the steering wheel was no longer centered. The contact was able to continue to drive the vehicle. The contact stated that the failure reoccurred while driving and the warning message remained illuminated. The dealer was notified of the failure and advised the contact that the vehicle could not be repaired under NHTSA Campaign Number: 10V218000 (Steering) because the recall had expired. The manufacturer was not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 120,000.

THE CONTACT STATED THAT SEVERAL UNKNOWN WARNING LIGHTS WERE ILLUMINATED. THE VEHICLE WAS TAKEN TO WILKIE LEXUS (568 LANCASTER AVE, HAVERFORD, PA 19041) WHERE IT WAS DIAGNOSED WITH THE BRAKE ACTUATOR NEEDING TO BE REPLACED. THE VEHICLE WAS NOT YET REPAIRED. THE MANUFACTURER WAS INFORMED OF THE FAILURE AND OFFERED TO COVER 20% OF THE PARTS COST. THE FAILURE MILEAGE WAS APPROXIMATELY 121,000.

    TL* THE CONTACT OWNED A 2010 LEXUS LS460. WHILE DRIVING 55 MPH, THE VEHICLE BEGAN TO SKID ON THE FREEWAY. WHEN THE CONTACT DEPRESSED THE BRAKE PEDAL, THE VEHICLE FAILED TO STOP. AS A RESULT, THE VEHICLE CRASHED INTO A SIDE EMBANKMENT, BOUNCED BACK ONTO THE FREEWAY, TURNED 360 DEGREES, AND CRASHED INTO THE EMBANKMENT AGAIN. THE VEHICLE THEN BOUNCED BACK A SECOND TIME ONTO THE FREEWAY BEFORE IT CAME TO A FULL STOP. BYSTANDERS HELPED TO COAST THE VEHICLE OVER TO THE SIDE OF THE ROAD. THE AIR BAGS FAILED TO DEPLOY. THE DRIVER SUSTAINED NECK AND BACK INJURIES THAT DID NOT REQUIRE MEDICAL ATTENTION. THE FRONT PASSENGER SUSTAINED A SPINAL FRACTURE, EYE SOCKET FRACTURE, AND BACK INJURIES THAT REQUIRED MEDICAL ATTENTION. THE REAR PASSENGER SUSTAINED HEAD INJURIES THAT DID NOT REQUIRE MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS TOWED TO THE CONTACT'S RESIDENCE. THE VEHICLE WAS NOT TAKEN TO A DEALER OR AN INDEPENDENT MECHANIC FOR DIAGNOSTIC TESTING. THE VEHICLE WAS DESTROYED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 75,000.

    I AM EXPERIENCING AN ANTI LOCK BRAKES ACTUATOR FAILURE. UPON STARTING THE VEHICLE IT GIVES THE WARNING "STOP THE VEHICLE AND CONTACT A DEALER". I HAVE DID THIS AND FOUND THAT THIS IS AN ISSUE THAT TOYOTA/LEXUS IS AWARE OF AS THEY HAVE ISSUED A SERVICE BULLETIN ABOUT IT AND OFFERED AN EXTENDED WARRANTY. HOWEVER, THE EXTENDED WARRANTY IS LIMITED BY MILEAGE AND YEARS. BEING A KEY SAFETY COMPONENT IN VEHICLE OPERATION, I AM REQUESTING THIS ISSUE BE CONVERTED INTO A RECALL. I HAVE TRIED TO GET REPAIR ASSISTANCE FROM LEXUS CUSTOMER SUPPORT AND THEY DENIED THE REQUEST BASED ON MILEAGE.

LEXUS OF WESTMINSTER IN WESTMINSTER, CALIFORNIA DIAGNOSED THAT THE BRAKES WERE WORN. THE BRAKES WERE REPLACED, BUT THE FAILURE RECURRED. AUTO CLINIC OF IRVINE, CALIFORNIA, A NON-LEXUS DEALER, PERFORMED A DIAGNOSTIC TEST AND DETERMINED THAT THE VEHICLE WAS INCLUDED IN TSB: L-SB-0036-15 REV, BUT THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE ISSUE AND STATED THAT, ALTHOUGH THE VEHICLE WAS REPAIRED ONCE BY A LEXUS DEALER, A LEXUS DIAGNOSTIC MUST BE PERFORMED ON THE VEHICLE. THE CONTACT STATED THAT THE VEHICLE WAS UNDER WARRANTY FOR 100,000 MILES UNTIL FEBRUARY 28, 2016 OR UNTIL THE MILEAGE CAP WAS REACHED. THE FAILURE MILEAGE WAS APPROXIMATELY 40,000.

THE RPMS INCREASED AND THE VEHICLE MADE A GROWLING NOISE, BUT DID NOT ACCELERATE. THERE WAS NO WARNING INDICATOR. THE FAILURE RECURRED SEVERAL TIMES AND THE VEHICLE WAS TAKEN TO A LOCAL DEALER WHERE IT WAS DIAGNOSED THAT THE ISSUE WAS NORMAL FOR A NEW VEHICLE. THE FAILURE RECURRED AND THE VEHICLE WAS TAKEN BACK TO THE LOCAL DEALER WHERE IT WAS DIAGNOSED THAT THE FAILURE WAS DUE TO THE TYPE OF FUEL BEING USED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND ORDERED THE LOCAL DEALER TO PERFORM EXTENDED DIAGNOSTIC TESTING ON THE VEHICLE. THE DEALER WAS UNABLE TO REMEDY THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 28,000.

NEXT THING THE BRAKE DIS-ENGAGED AND CAR LUNGED FORWARD HITTING THE CAR IN FRONT OF ME AND THE CAR IN FRONT OF THAT VEHICLE BEFORE I HAD TIME TO HIT THE BRAKE. DAMAGE TO LEXUS IS $7000 NO ESTIMATES ON DAMAGE TO OTHER CARS. OTHER DRIVERS REPORTED AND GETTING MEDICAL ATTENTION.
